Planning Your First Visit

As a patient, there are a few things you can do to make the most of your visit with Morgan Integrative Health. The better prepared you are, the more effective your visit will be.

1. Complete the new patient form.
If you are a new patient, it is very important to complete this comprehensive form in our online TherapyPortal ahead of your appointment. This will give us important information to better understand your health situation.

2. Bring your medications and supplements for your first office visit.
Gather all of your current medications and supplements and bring them with you.

3. Bring a list of questions.
Keep an ongoing list of questions between visits that you would like to ask. Keep note of any changes you may be experiencing. Write down symptoms, duration of the condition, frequency, and so on.

4. Consider bringing a family member or friend with you to your appointment.
Feel free to bring a friend or family member with you. You will likely be receiving quite a bit of information and it can be very helpful to have another set of ears to help you remember the details about your visit. Of course, you are always encouraged to call our office with any questions or concerns.

5. Call or email us with any questions or concerns.
Optimal healthcare involves addressing the body, mind, and spirit. By listening carefully to our patients and utilizing modern diagnostic testing techniques, our health care professionals are able to pinpoint and treat the root cause(s) of a person’s health problems. But good health does not just happen. You must take an active role. Ask questions and be open and honest with your provider and they will help you be the best you can be so you can enjoy your life to the fullest.
